The ship did not ground, she broke in two on the open sea from failure of the hull in heavy weather

Still a tragic end for all concerned.

She had left Rotterdam(?) insufficiently ballasted to avoid de-ballasting costs in the Great Lakes. She slammed heavily in very bad weather across the Atlantic, and finally broke in two with the crack initiating at a grain loading port. The enquiry also noted poor maintenance, unqualified welding repairs, and failure to rectify relevant Class noted defects.
